Diversified Health Care will automatically bill several major insurance companies. Insurance claims are completed as a courtesy to you, without charge.

  • Co-pays and Deductibles are due at the time of the appointment. In the event that you have a secondary, we must still collect your co-payment or deductible at the time of the appointment.

  • You are responsible directly to the practice for payment of your account within the time limit set, as a courtesy; claims are filed to Primary insurance companies only. In the event of no response from your insurance carrier within forty-five (45) days, you will be billed for the balance due.

  • If you provide us with incorrect insurance information and your claim is denied, you will be required to pay the balance due and resubmit it to the correct insurance company.

To better serve you, if you call the office after hours regarding canceling or scheduling appointments, refills or referrals and other non-emergency requests please wait and call the office during normal business hours.
Please be sure to maintain an accurate phone number with the office. In the event of inclement weather or sudden closure the office will send out automated messages of the closure. Once the office re-opens one of our team members will contact you with your new appointment date and time.
